The tessera sanitaria (health card) and the tax code

How do I request a duplicate card?

If your plastic card or certificate is lost, stolen or damaged, file a request at a Revenue Agency office or through its online services. You will receive a new card or certificate; your Codice Fiscale remains unchanged.

Do I have to update the code when I move?

The code itself doesn’t change when you change residence. However, you should inform the Revenue Agency of your new address so that your records and health‑card mailing address are updated.

What happens if I change my name or marital status?

If your surname changes due to marriage or other legal reasons, you must notify the Revenue Agency using Form AA4/8 to obtain a revised code. The new code reflects your updated name but keeps the same birth‑date and place components.

Does the health card replace the tax code card?

Yes. The health card contains the tax code and serves as proof of the Codice Fiscale. If your health card expires, the tax code printed on it remains valid; you can still use it as proof.

Who receives the health card?

People residing in Italy and entitled to the National Health Service automatically receive the health card when their Codice Fiscale is issued. The card is sent to the address recorded with the Revenue Agency. Newborns get it after being registered and assigned a code.

What is the Italian health card (tessera sanitaria)?

The tessera sanitaria is a plastic card that contains your Codice Fiscale, personal details and health‑insurance information. It doubles as your National Health Service card and as your European Health Insurance Card within the EU.
